The purpose of this study was to evaluate the ability of fluoride by means of fluoride iontophoresis to inhibit or reverse orthodontically related demineralization. Twelve orthodontic patients (12-16 years old) scheduled to have premolars extracted were randomly divided into two groups - fluoride one application and fluoride two application groups. The premolars (44 teeth) were bracketed using an acid-etch composite system; each patient was given precise oral hygiene instruction and supplied with an orthodontic toothbrush and nonfluoride containing toothpaste. 2.0% Sodium fluoride solution was applyed for 5 minutes by means of fluoride iontophoresis equipment. Fluoride 1 group received only one application of fluoride on the upper and lower premolars on one side, and the premolars of the other side were control 1. Fluoride 2 group received fluoride application two times at one week intervals on one side premolars and. the other side was adapted as control 2. All premolars were extracted after 4 weeks. Each tooth was sectioned, embedded in unsaturated polyester arid polished with silicon carbide paper (#1200 mesh). The slabs were tested using the Vicker¢¥s microhardness tester (MXT- a 7) at three points from bracket base.
Results showed that the Vicker¢¥s hardness numbers were higher in both fluoride 1 and fluoride 2 groups compared with the control groups (p<0.05). Surface microhardness values were smaller closer to the bracket base (p<0.05). Fluoride 2 group showed higher hardness value increases compared with the fluoride 1 group, but there was no statistically significant difference (p>0.05).
It was concluded that (1) measurable demineralization occurred around an orthodontic attachment after only 4 weeks and (2) this demineralization can be effectively inhibited and/or reversed by the use of fluoride iontophoresis.
